In connection with my inquiry, application, transaction, purchase, negotiation, loan, sale or event participation with GENESIS CAPITAL FINANCING INC., I hereby authorize GCFI to process my personal information, sensitive personal information and privileged information, otherwise known as my Personal Information or Data, in compliance with Data Privacy Act of 2012 and its Implementing Rules and Regulations and GCFI’s Manual and Privacy Policy. By signing this consent form, I hereby agree and consent that GCFI may collect, record, organize, store, update, use, consolidate, block, erase, share or otherwise process information, whether personal, sensitive or privileged provided by me for any of the following purposes:

1. Processing of Sales and Service Transactions, including but not limited to inquiries, loan, sale of motor vehicles and/or spare parts, service transactions or any other related transactions with GCFI;
2. Sharing of Data to GCFI’s Accredited Dealerships, subsidiaries, affiliates, bank partners, suppliers and contractors for the proper handling of loan, sales inquiry, sale of vehicle, spare parts, accessories and servicing of vehicles.
3. Participation in GCFI’s marketing, digital marketing and promotional activities, including but not limited to marketing events, promotions, contests and invitations to company events;
4. Posting of my pictures taken in connection with my transactions with GCFI for marketing, promotional activities or company events;
5. Dealer or Job applications, processing of employment with GCFI, employment background investigation, financial capacity investigation, customer satisfaction survey, contractual and service obligation, and supplier’s accreditation;
6. Other transactions with GCFI that requires giving of Data.

I hereby approve the disclosure of my Personal Information acquired by GCFI to the following:
1. To its third-party service providers, government agencies, contractors and sub-contractors and GCFI’s subsidiaries and affiliates; should the sharing of my Data be necessary in connection with or to render effective my possible employment, communication, contractual obligations or transactions with GCFI or in cases filed by or against GCFI;
2. Bank Partners, Insurance Companies and/or Government Agencies in connection with sales and service transactions with GCFI;
3. GCFI’s Accredited Dealerships.

I hereby acknowledge that by signing this consent form, I am giving GCFI the right to store my Personal Information in their Data Base (Physical or Electronic) for a period of five (5) years from the last transaction or communication with GCFI, except when retention for a longer period thereof is necessary. My Personal Data may be used in sending me event or Marketing Invitations, promotions, email notifications and other activities conducted by GCFI and its authorized Dealerships. I am fully aware that upon the lapse of five (5) years from my last transaction or communication with GCFI, my Personal Data shall be destroyed, disposed or discarded in a secure manner that would prevent further processing, unauthorized access, or disclosure to any other party or the public. I reserve the right to withdraw my consent at any time in processing and use of my Personal Information or correction of my data upon proper notice to GCFI through mail or email at dpo@genesisfinance.ph, however, by withdrawing such consent, I hereby recognize the right of GCFI to cancel any business or transactions in connection with me.
